Chronic Obstructive Pulmonary Disease (COPD): Clinical Sciences

Key Concepts

  • COPD Characteristics: Chronic respiratory symptoms due to progressive airflow obstruction.

    • Causes include airway inflammation (chronic bronchitis) and alveolar wall destruction (emphysema).
  • Primary Causes:

    • Tobacco use is the leading cause.
    • Other causes: secondhand smoke, environmental exposures, alpha-1 antitrypsin deficiency.

Clinical Presentation

  • Symptoms: Shortness of breath, chronic cough, wheezing.
  • Progression: Can range from mild exacerbations to severe COPD exacerbations.
  • Complications: Bacterial pneumonia, pulmonary hypertension, acute respiratory failure.

Diagnosis

  • Assessment: Perform an ABCDE assessment to determine patient stability.

    • Unstable Patients: Look for alarm signs such as worsening dyspnea, purulent sputum, cyanosis.
    • Stable Patients: Focused history and physical exam for wheezing, hypoxemia, tachypnea.
  • Spirometry Testing:

    • FEV1/FVC Ratio: Key for COPD diagnosis.
    • Less than 0.7 confirms COPD.
    • Use spirometry to differentiate between restrictive and obstructive lung diseases.

Treatment

  • Pharmacologic Therapy:

    • Stage 1 (Mild): Inhaled bronchodilators like SABAs or SAMAs.
    • Stage 2 and Beyond: LABAs, LAMAs, or combination therapy.
    • Advanced Stages: May include inhaled corticosteroids, PDE-4 inhibitors, macrolide antibiotics.
  • Non-Pharmacologic Therapy:

    • Lifestyle modifications such as smoking cessation.
    • Vaccinations: Influenza and pneumococcal.

Management Strategies

  • Initial Stabilization:

    • Airway, breathing, circulation management.
    • Possible mechanical ventilation, IV access, vital sign monitoring.
  • Ongoing Management:

    • GOLD staging based on FEV1 percentage: guides treatment and management of COPD.

Clinical Pearls

  • COPD patients should have formal GOLD grade and stage assessment for treatment implications.
  • Regular lifestyle counseling and vaccinations are crucial to management.
