Lecture on HIV Coding Guidelines

Introduction to HIV

  • HIV: Acronym for Human Immunodeficiency Virus
    • Attacks cells that help the body fight infection.
    • No cure, but treatable with medication.
    • Can be undetectable with treatment.

Key Guidelines for Coding HIV/AIDS

  • B20 (HIV AIDS) Code
    • Once assigned, no other HIV-related code should be used.
    • Codes to exclude once B20 is assigned:
      • R75: Inconclusive laboratory evidence of HIV
      • Z20.6: Contact with and suspected exposure to HIV
      • Z11.4: Encounter for screening for HIV
      • Z71.7: HIV counseling with a negative result and no symptoms
      • Z21: Asymptomatic HIV
      • 098.7: HIV disease complicating pregnancy, childbirth, and the peripartum period.

Sequencing Guidelines for HIV/AIDS

  • HIV/AIDS encounter related condition: Sequence HIV/AIDS (B20) first.
  • Encounter not related to HIV/AIDS: Sequence the reason for the encounter first, followed by HIV/AIDS code (B20).

Quiz on HIV Codes

  1. B20: HIV AIDS code for patients with diagnosed HIV/AIDS.
  2. R75: Inconclusive lab evidence of HIV.
  3. Z20.6: Contact/exposure to HIV.
  4. Z11.4: Screening for HIV.
  5. Z21: Asymptomatic HIV.

Coding Scenarios

  • Scenario 1: 32-year-old pregnant woman with controlled gestational diabetes, asymptomatic HIV.
    • Sequence: 098.712 -> B20 -> Gestational code -> Asymptomatic code.
  • Scenario 2: 35-year-old male with symptoms leading to anxiety, possible HIV, elevated BP.
    • Sequence: F41.9 (Anxiety) -> Symptoms integral to anxiety.
  • Scenario 3: 45-year-old AIDS patient with symptoms (sore throat, cough), diagnosed with seasonal allergies.
    • Sequence: J30.2 (Allergies) -> B20 (AIDS).
  • Scenario 4: Pregnant woman with AIDS presenting symptoms.
    • Sequence: 098.713 -> B20 -> HIV related condition.
  • Scenario 5: Pregnant woman, HIV positive, diagnosed with pneumocystis pneumonia.
    • Sequence: 098.713 -> B20 -> Pneumocystis pneumonia -> Week's gestation code.

Guidelines for Conducting Scenarios

  1. Inventory Key Terms:
  • Diagnosis, signs, symptoms.
  1. Identify First-listed Code:
  • Apply sequencing rules.
  1. Apply All Guidelines:
  • Especially for pregnancy and HIV-related conditions.

General Reminders

  • Categories On-site:
    • Infectious diseases: A00-B99.
    • Remember Primary Sequence Codes for Chapter 15 (Pregnancy Codes: O).
  • Sign and Symptom Coding:
    • Code signs and symptoms first if no diagnosis is present.
    • Do not code signs and symptoms integral to a diagnosis.

Summary

  • HIV Coding requires adherence to specific guidelines.
  • Coding Order: HIV/AIDS impact the sequencing of codes.
  • Practice Scenarios: Essential for understanding and applying guidelines.

Remember to constantly refer to guidelines and updates to ensure accurate coding and documentation.
