OCT 720: Week 5: Shoulder Complex Part II

okay so let's look at the muscle group that contribute to uh the scab movement for scab elevation um here's what I U typically um um um do that I I really look at the origin and insertion uh at this uh region that help me to understand uh or memorize those muscles function for example levator scapula um it originated from spine and kind of insert at the superior ankle of the scapula right here so when the muscle contract it can of lift up on the scabo and so um this the upper uh trapezius you can see the muscle fibers right here so uh the origins from spine and the insertion is um at the um um um scapula so when the muscle contract it can to elevate um the scapula okay and for depression um scab depression again if you look at the muscle um we have lower trapezius kind of pull the scab down we have the tus door sign again uh it doesn't really attach to the scaba but when the G joint kind of do adduction that uh to something degree that would help to uh for the uh scab depression and if you look at uh look at the the angle uh from the front you see the P major uh also contribute to this scab um depression because it can help to um add up the GH joint okay and the scapular depression typically U we um it's uh working with gravity okay because it's it's moving in the in the direction that uh with uh the gravity pool so um when we do uh walking with crotches then we going to push off that bar then um the and we're doing um Against Gravity movement for the scab depression so another example for um Against Gravity uh for uh scav depression is when we do um the sliding board transfer so we ask our patient to push up their body so that they can you know Glide or slide on the um on the transfer board that's one uh example of um scar depression working against gravity okay so um the scab upward rotation it's really a combination of mus of several muscles we talk about this example uh earlier uh this semester uh for uh scaba turn upward we need upper trapezius to put this way and lower chapeus to pull uh to pull the gabula down and then along with the seratus interior to pull it um outside or laterally together these three uh muscles um produce the scab upward rotation and for scav downward rotation again you uh we want to look at the muscle the direction the muscle um origin and insertion so uh if you look at the muscle right here from the back um um the rboy minor and Ron boy major kind of pull uh when when the muscle contract that will pull the the uh scaba a little bit downward okay and for p minor from the uh front you see it kind attach to uh the chical process right here so when the muscle contract you going of pull the scaba downward so again um combined the three muscles they reduce uh the scab downward uh rotation and you may find it hard to to memorize the rmo minor or rboy major I use this um spine of the scaba um to to help me so above the spine that's minor below the spine that's major and you can also look at the the size of the muscle you can kind of see why we call it a major and this is a minor okay um scapular protraction um well it's it's not um I think we are very familiar with this but if you if you look at the the seratus interior uh originated from the rib and going to reach to the medial border of the scapula when the muscle uh contract it going to bring the uh scula laterally and a little bit forward you kind see going to move along the thoracic um region okay and because of this pull naturally on the mle border we're going to going to go up a little bit because it's going to move along with the rib cage so uh we also call it a winging um it's it's normal however however um uh if there's too much winging um showing up then that will indicate a weak seratus anterior okay and people with long thoracic U nerve injury may also show this because um the long thoracic nerve innovates the cectus interior as well okay um and then the scapular retraction if you look at the muscle right here uh we need uh a rmo minor and major and then we also need the middle trapezius because if you look at the muscle fiber it kind of pull um the scab toward uh the middle okay so these are um the muscle group kind of working together for the shoulder girdle we U talk about elevation depression um protraction retraction downward rotation um and upward rotation okay
