all right first thing we're going to do as soon as we have the patient we're going to check um doctor's orders we're going to check what type of IV if they have an IV if it makes sense we're going to check the gauge as well as the size of the needle we're also going to have to check the patient's history the diagnosis to make sure it correlates with the IV cath insertion we're also going to have to check for any allergies and just any other pertinent information that we need to know before we do an IV insertion okay and then after we've done that we're going to gather our materials we need an I start kit in the IV start kit you're going to have color hexidine and then you're also going to have your dressing change label going to have your tape your gauze your tourniquet you won't have an adapter or a clave in there but you must get one and it also must have a cap and this should be sterile all time so just remember that you'll also need some alcohol prep pads and that should be it make sure you organize your materials efficiently make sure you keep them in here before you visit the patient's room make sure you also have your tagm and make sure when you gather your needle make sure you have the expiration date make sure it's not expired make sure it's not tampered with it's not broken or anything doesn't look out of the ordinary same thing with this you're Tagg in make sure it's not um expired or anything like that and all of your other materials as well and with the saline flush I can go ahead open it make sure it doesn't have a strange color no yellow discoloration or anything also check the expiration date on this guy as well also make sure I just opened it but that it's unopened also going to grab a check as well and after you have all of your proper materials and you've received report on the patient you can go ahead hit to the patient's room hand hygiene first knock on the door go ahead and go inside of the room greet the patient check do a general survey check for ABC's check for B uh bed safety as well make sure that patient is Alive and Well introduce yourself hi my name is your nurse and I'm going to be your nurse for today and you're going to have to do hand hygiene put on some gloves while you're doing this go ahead and just like I said with your General survey make sure you ize that once more in case you forgot for some [Music] reason make sure both bed rolls are up because I've seen one bed ra down during testing make sure the bed is also down to the lowest position you could go ahead identify your patient may have your full name date of birth and then after that you can go ahead and check for allergies make sure you specifically ask for allergies to latex chlorexidine beta iodine adhesive and yeah that's it we're good after you've identified your patient go ahead ask a couple of questions that you need to know if they had a masectomy also if they had um a lymp lymphadectomy also ask for a stent or fistula or if they had a prior history of stroke cuz all of those are counter indications also you're going to want to ask for a preference on which arm they would rather prefer that they tell you left or right make sure you go to that arm and also after that that's pretty much it you can start with your raising the bed also position it while you're lifting it up to waist height position it to low fowers position it to low Fowlers so just make sure it's at low fowers that's good so after you have your patient's arm can go ahead put the cheuck check the arm make sure you don't see any discoloration or anything no bruises anything that would otherwise hinder your potential IV insertion site good I missing unroll your tourniquet [Music] go place your turn on your patient 3 to four Ines above your insertion side go like this this guy you're going to overlap we going to tuck it underneath like so and then after go ahead ask them to clench their fists when they clench their fists their veins will come a little bit a little bit more prominent if needed you can have them hang their limb off of the bed and just go ahead you're going to feel this shouldn't be but just make sure you're feeling for a nice insertion site so I think I have a good vein right here go ahead pop this guy off right go ahead grab your alcohol prep pad you're going to do and you have to verbalize this vigorous Center and then move outward go ahead discard this let that dry and while you're letting that dry you can go ahead and prepare your supplies go ahead UNC cover this everything in here is sterile so don't go in there touching taking stuff out and then putting it back in there you will fail make sure you have everything ready open up your tag make sure to keep a nice St area you don't want your supplies scattered everywhere go ahead clean up anything that should be trash go ahead and lay down your tagm right here it's probably better to do this just have your supplies on here so that way it's nice and clean along with your adapter if for some reason you touch it right here make sure you verbalize that you either get a new clave or you just go ahead and clean it with an alcohol prep pad okay and then here's the part you need to make sure that this guy you prime it just go ahead get rid of all those air bubbles try not to get water everywhere like I just make sure main idea just get all your bubbles out okay go ahead you can put your clave on this guy be very careful if you touch any of your ports just say that you will get a new saline or same thing with the saline just make sure you get a new one or you disinfect it you cannot do that with the saling just got a new one but for the cave you can disinfect it keep this guy in your sterile area all righty now after that patient arm should be nice and dry and go ahead put your tourniquet back on same way I told you this guy over tuck it under 3 to 4 in above your insertion site don't touch it cuz if you touch it uh you'll fa right and then after go ahead grab your needle uncap you can trash this check bevel up check make sure there's no rust It's Not Bent or anything out of the ordinary they usually it goes from instructor to instructor they say to push this up a bit personally I wouldn't but you can go ahead insertion site is right there you're going to go bevel up skin toed 10 to 15° going to go in righty as soon as you see that flash of blood right there go ahead start advancing and withdraw your needle advance in one smooth motion okay one nice smooth motion perfect do not touch that insertion site try to keep your hand off of it and just pull this guy the remainder out pop him off pop your tourniquet immediately if you don't um I don't know you'll fail throw this guy in your Sharps go ahead keep placement right right here after you're going to grab your tagm it varies they say to flush first personally I think it's safer to have that stability before you flush so you can go ahead just keep holding you could put your middle this is an easy way this how to do it you put your middle finger don't touch the insertion side keep two hands right here and simultaneously with your thumb pull it be very careful drop that guy make sure it doesn't touch your insertion site and you're going to lay it kind of like a blanket make sure that you have good area of coverage yeah that's good all right again don't take tuck start tucking start tucking keep going and then you can let go and then just start tucking even more try to get as rid many air bubbles as you can after you reach this point pull your green flap and go ahead keep tucking get rid of as many air bubbles as you can make sure you verbalize that and also verbalize that it's you're doing that in order to get rid of any bacteria and after that reveal that guy we're almost done keep tucking keep tucking all righty after that just make sure it looks nice and clean you can go ahead grab it right here clam go ahead take this guy and then unclamp and all you really need to do with this guy is just flush it you don't need to withdraw or pull any fluid from here to see blood just flush and while you're flushing clamp to create negative pressure you can go ahead take your SA enough don't allow anything to touch that and then you can and then do you have one over here there thought I had one and then make sure you have an unopened curos cap you can go ahead open it just bring it off to the side open that guy and then put it on here make sure you don't let anything touch your clay and then after 3 minutes It'll be completely sterile have that and then if needed you can go ahead and clamp this guy move it down so that way we have some space go ahead clamp that there we go then you can grab your tape make sure to clean up your space obviously all right you almost finished this one I think I still I got it I got it ready grab like I would say about this length that's inappropriate one right go ahead place this guy right here careful not to put it on the tubing cuz you can Kink it just going to want to fold it over like this to create some slack go ahead and make sure it's on an appropriate side you don't want to have it down here to where it can hit the patient body right about here should be fine make sure there's no kinks in your tubing go ahead Lay It tape a and tape it all right and the most important part date time initial on this guy you go ahead put it somewhere appropriate right here maybe not cuz you can't really see possibly right here you don't want to obstruct it like that that's just don't do that okay but you just want to put it put it in an appropriate place for that right after that you've already assessed patency with your patient's catheter you've already flushed it you saw it's pent and went through other stuff you're going to make sure that the patient tolerated well you want to ask them how they felt after you're going to go ahead and check make sure everything else is clean and neat you want to educate your patient the most important thing you need to tell them to look out for signs of infiltration that's going to be a swelling over the arm of the IV insertion site along with some coolness it's going to be uh they're going to have a Dem but they won't know that so make sure you just tell them to look out for that also for fitis you're going to see redness and then irritation in this general area also tell them to keep an ey for that and most important part to call you on their call light all righty after that just clean up your area you know nice and tidy that's the whole point of nursing after that you can goad editation I did oh I after that you can go ahead just make sure that they're comfortable check for bed safety lower the bed to the lowest position make sure we've placed them in low Fowlers make sure they're in that position if they're having breathing if they're having any difficulty breathing raise them up after that bump the bed you state your name my name is your nurse all righty make sure they have their call light and go
