Calculate Expected Date of Delivery (EDD)\n1. Calculate EDD with Last Menstrual Period (LMP):

  • Formula: Add 9 months and 7 days to the first day of the last menstrual period.
  • Example: LMP = December 27, 2021
    • Add 7 days: January 3, 2022
    • Add 9 months: October 3, 2022
  • EDD: October 3, 2022.
  1. Consider Adding for February:

    • For February, always add 9 months first and then 7 days.
    • Example: LMP = February 25, 2021
      • Add 9 months: November 25, 2021
      • Add 7 days: December 2, 2021
    • EDD: December 2, 2021.
  2. IVF EDD Calculation:

    • Fresh Cycle: Add 266 days from the retrieval day.
    • Frozen Cycle: 263 days for day 3 transfer; 261 days for day 5 transfer.

Obstetrical Score (GTPAL) Overview

  1. Definitions:

    • Gravida (G): Number of pregnancies (current + past).
    • Pariy (P): Number of pregnancies beyond 28 weeks (term).
  2. Writing Obstetrical Score:

    • Format: GTPAL (G#:T# P# A# L#)
      • Example: G5P2A1L3 indicates a woman who has been pregnant 5 times, 2 term deliveries, 1 abortion, and currently has 3 living children.
  3. Components:

    • Term pregnancies (T), Preterm pregnancies, Abortions, Living children.

Types of Pregnancies and Complications

  1. Post-Term Pregnancy:

    • Associated with:
      • Macrosomia
      • Variable deceleration on CTG
      • Meconium aspiration (not transient tachypnea of the newborn).
    • Management: Induction at 41 weeks.
    • Characteristic amniotic fluid color: Greenish-yellow, saffron.
  2. Preterm Labor Management:

    • Early Preterm (<34 weeks): Give corticosteroids + tocolytics.
    • Late Preterm (34-36 weeks): Corticosteroids only.
    • Tocolytics: Indicated only if labor is before 34 weeks.
  3. Important Terms:

    • GBS Screening: Recommended at 36-37 weeks.

Nutritional and Supplement Guidelines

  1. Caloric Requirements:

    • 1st Trimester: 85 kcal/day.
    • 2nd Trimester: 300 kcal/day.
    • 3rd Trimester: 450 kcal/day.
  2. Folic Acid Supplementation:

    • Average intake: 400 mcg/day, 4 mg if prior neural tube defect.
  3. Routine Daily Allowances (RDA):

    • Iodine: 250 mcg/day.
    • Calcium: 1000 mg/day.

    • Indication: Used to assess readiness for labor induction; score of 8 or higher indicates favorable conditions.
