Nursing Procedure: Applying Soft Wrist Restraint

Introduction

  • Nurse: Miss Hartman
  • Objective: Apply a soft wrist restraint on the left arm

Steps for Application

  1. Placement

    • Apply restraint according to manufacturer's instructions.
  2. Alignment

    • Maintain the restrained extremity in normal body alignment.
    • Purpose: Prevent contractures and neurovascular injury.
  3. Adjustment

    • Pull the tie around/through the restraint.
    • Ensure two fingers can fit between restraint and wrist/ankle.
    • Purpose: Avoid impaired circulation.
  4. Securing the Restraint

    • Secure tie around a moving bedframe (when head of bed is adjusted).
    • Use a half bow or slip knot.
    • Note: Do not attach to side rails.
    • Knot type ensures easy untying by pulling the loose end.
