Role of a Home Health Nurse

Hi, my name is Amber and I'm a registered nurse with Vallejo Home Health. On each day, I provide a lot of education to my patients on various topics. I perform health assessments. Usually on the first visit, it's the more thorough assessment. We sit down and we go over health history, and then I will do a thorough assessment, listening to their heart, their lungs, doing a skin assessment. checking out the whole picture so that way we can come up with a game plan on how we're going to improve their health. Medication management is a big one and we have a lot of diabetic patients, so we help manage diabetes, any heart disease, we help manage that also. We also have a lot of therapists on board, so we kind of work as a team to try to get strength and energy back if somebody has had a fall in the home setting. A lot of my patients don't realize that we can actually draw blood in the home. We can take the lab to whatever. facility their insurance covers so they don't have to go to the doctor's office to get their blood drawn. We can take any sort of lab like a urine sample to the lab and they don't have to leave their home. Another thing that a lot of people don't realize that we can do in the home setting is wound care. They think that they have to travel to the doctor's office multiple times a week to get their dressing changed but we cover that so that way they don't have to leave their home. It's perfect for patients that may may have a hard time leaving their home, it might require a lot of effort or they're unsafe to leave their home by themselves. And so we come to them and can provide most of the cares that the doctor's offices can provide we can do in the home. They still do need to go see their doctors for follow-up or just checkups, but we can provide a lot of different things in the home setting that way they don't have to leave their home. It just depends on what the patient needs. needs. So sometimes it's a quick 30 minute visit, sometimes it's an hour, but I make sure that I spend my time with my patient. That's one of my favorite things about being a home health nurse is I actually get the time that I need with them. I really get to know my patients. I get to know their family and I love seeing my patients each week. We kind of form a professional relationship. I love that I get to help them improve. I love it. and by the time we discharged them, they're doing a million times better than the first time I saw them. So I like to see that growth and that improvement.
