How to Report Violations
- LVNs or PTs can report misconduct by:
- Completing the Licensee Mandatory Reporting Form (available on the Board's website).

Reporting Requirements
- Failure to report known violations of LVN or PT laws by another licensee is considered unprofessional conduct.
- Licensees must report violations to the Board in writing within 30 days of awareness.
- Must cooperate with the Board in providing information or assistance.
Reportable Violations
- Unprofessional conduct by an LVN or PT, including:
- Incompetence or gross negligence in nursing functions
- Excessive force or abuse of patients
- Failure to maintain patient confidentiality
- Dishonesty related to professional duties
- Failure to follow infection control guidelines
- Illegal possession or use of controlled substances
- Illegal drug/alcohol use affecting public safety and practice
- Court confinement for addiction
- Falsified or inconsistent entries regarding controlled substances
