How to Report Violations

  • LVNs or PTs can report misconduct by:
    • Completing the Licensee Mandatory Reporting Form (available on the Board's website).
    • Writing to the Board's Enforcement Division at:

Reporting Requirements

  • Failure to report known violations of LVN or PT laws by another licensee is considered unprofessional conduct.
  • Licensees must report violations to the Board in writing within 30 days of awareness.
  • Must cooperate with the Board in providing information or assistance.

Reportable Violations

  • Unprofessional conduct by an LVN or PT, including:
    • Incompetence or gross negligence in nursing functions
    • Excessive force or abuse of patients
    • Failure to maintain patient confidentiality
    • Dishonesty related to professional duties
    • Failure to follow infection control guidelines
    • Illegal possession or use of controlled substances
    • Illegal drug/alcohol use affecting public safety and practice
    • Court confinement for addiction
    • Falsified or inconsistent entries regarding controlled substances
