8D Problem Solving Methodology

Overview

  • Purpose: Identify, correct, and eliminate recurring problems.
  • Developed by: Ford Motor Company in the late 1980s.
  • Use: Document problem-solving efforts, primarily in product and process improvements.

8 Disciplines (Steps)

D0: Plan

  • Plan and prepare for the 8D process.
  • Identify necessary people and resources.
  • Proper planning prevents future problems and saves time and money.

D1: Team Formation

  • Form a cross-functional team.
    • Include members from R&D, Quality, Operations.
    • Critical members: Leader and Champion/Sponsor.
  • Ensure the team has the required skills and knowledge.

D2: Problem Description

  • Describe the problem using known data.
  • Use the 5W1H approach (Who, What, When, Where, Why, and How).

D3: Contain the Problem

  • Implement temporary actions to minimize the problem's impact on the end-user.
  • Containment is not a permanent solution; it differs from corrective actions.

D4: Identify Root Cause

  • Focus on identifying the root cause once the problem is temporarily contained.
  • Tools used:
    • 5 Whys
    • Fishbone/Ishikawa Diagram
    • Pareto Chart

D5: Corrective Actions

  • Determine the best long-term solution.
  • Use brainstorming and tools like the Affinity Diagram.

D6: Validate Corrective Actions

  • Ensure corrective actions are effectively implemented.
