Serial funal height measurements are easy to perform, inexpensive and non-invasive. It is a screening tool which is acceptable to the mother. The technique we recommend is as follows. Before you begin, explain the procedure to the mother and gain her informed consent. You will need a non-elastic tape measure which begins at 0 cm. Ensure the mother has an empty bladder for the examination. A full bladder may add an additional 2 to 3 cm to the measurement. The mother should be positioned on a firm couch or mattress in a semi-reumbent position. If this is difficult, for example, if the mother is being examined at home and does not have a suitably firm bed, it may be necessary to ask her to lie on the floor supported by pillows under her head and shoulders. Perform a full abdominal two-handed palpation to determine the lie, presentation, and position of the fetus and to locate the highest point of the fundus and the top of the symphysis pubis. The highest point of the fundus may not be in the midline but may be displaced due to antiflexion of the uterus or fetal position. Once the highest point of the fundus has been located, leave one hand in place and secure the end of the tape measure to the fundus. Then measure down to the top of the symphysis pubis. Measure only once and document findings on the mother's customized growth chart. Do not be tempted to reme-measure as cyst will introduce bias. If your findings demonstrate a deviation from the normal slope of the growth curve on the customized growth chart, then refer the mother for an ultrasound scan to determine the estimated fetal weight.
