Document Checklist

  • Photocopy of valid proof of immigration status.
  • Supporting identity documents (original/copy signed by guarantor if copies).
  • Provide any existing non-expired travel documents.
  • Fee: $120 for travel document, $260 for certificate of identity.
    • Payment methods: Online, certified check, or money order.

Submission Details

  • Send application by mail to designated PO Box or address in Ottawa.
