[Music] hi everyone this is Doc Ena and this is my video instructional for the OS of medical students in the proper way of doing a pelvic exam and papsmear to download my lecture deck please go to my press site TOA obig here are the steps in doing a pelvic exam with papsmear first of course is to explain the purpose and conduct of the pelvic examination and when explaining use language or vocabulary that is appropriate and understandable provide adequate privacy while the patient undresses or removes her clothes and once undressed first inspect the external genitalia after inspection we proceed with doing the speculum exam so we prepare all the materials that are needed we need a speculum that is appropriately sized for the patient we need gloves we need cotton pledgets or we can also use cyto brush or cyto broom we need a glass light and a fixative so glove the examining hand to be used for internal examination so if you're right-handed you glove the right hand if you're left-handed you glove the left hand of course you always have the option of gloving both of your hands grasp the handle of the speculum using the ungloved hand lubricate the speculum with water so first you separate the lamora using the thumb and middle finger of the examining [Music] hand insert the speculum in a diagonal manner to avoid the urethra and rectum once inside the vagina swing the speculum into mid position until the cervix is reached open the speculum gently and adjust it until it cups and brings the cervix into full view then lock the speculum once the speculum is in place examine the cervix in vaginal walls and Report any visualized vaginal discharge report it color consistency adherence to walls odor or presence of arthema of the cervix or vaginal walls next is proceed to collect sample for the papsmear for this video I will teach you the three coton pled technique so we obtain specimens from the endocervix the exocervix and the lateral vaginal walls so first you obtain sample from the endocervical canal and then smear it on a glass light next the exos cervix or the Ecto cervix again we smear it on the glass slide just beside the first smear and lastly the lateral vaginal walls this will be our third specimen and then smear it on the third part of the glass slide and then unlock the speculum and gently close the speculum remove the speculum in the reverse sequence label the slide and apply a fixative now we proceed with the internal examination so we must perform the gynecological exam in an organized and orderly manner so first we gently separate the labia mahora with the thumb and the fourth finger then introduce the examining fingers into the introitus place the non-dominant hand over the supr pubic area and examine the cervix and Corpus and then the left and right adexa then gently withdraw the middle finger and introduce it into the opening or the anal opening and conduct a rectovaginal exam upon completion of the rectovaginal exam with draw the examining fingers gently and inform the patient that the examination has been concluded in concluding the visit of course you have to tell the patient to get dressed and once dressed entertain questions from the patient answer all the patients questions adequately and of course thank the patient that's it for this video instructional thank you for watching and please don't forget to subscribe in my YouTube channel in Rabon and my WordPress site doino obig thank you
