The Nursing Process Steps

1. Assessment

  • Focus on understanding the client's health problems and overall well-being.

2. Diagnosis

  • Uses the information gathered during assessment to determine the priority problem.
  • Employs a "nursing diagnosis" rather than a medical diagnosis.
  • Example: Impaired breathing issue diagnosed as "nursing diagnosis" even if the medical diagnosis is pneumonia.

3. Planning

  • Develop a care plan based on the identified problem.
  • Set goals and specific actions or interventions to improve patient outcomes.
  • Determine desired outcomes and aim for measurable improvements.

4. Implementation

  • Known as the action phase.
  • Perform interventions to improve the patient's condition.

5. Evaluation

  • Assess whether the goals set in the planning phase have been met.
  • Determine if changes are needed and whether outcomes have been achieved.
  • Reassess and restart the ADPIE process if necessary.

Focus on Assessment

  • Assessment is foundational to effective nursing and involves critical thinking:
