Thomas Test Overview and Interpretation

This is going to be a video on the Thomas Test for hip flexor contractures. To perform the Thomas Test, have your patient in supine laying position and first check for excessive lumbar lordosis, which is usually present with patients with a tight iliopsoas. Then, ask the patient to bring his knee as far to the chest as possible and to hold it afterwards and ask him if he can feel a stretch in the right groin. With a positive test, you will see that the extended leg is lifting off the table, like in this case. You can even measure the angle at the knee as a reference. If the extended leg is not lifting off the table, but it abducting, we are talking about the so-called J-Sign or stroke, which is indicative for tight iliotibial band.
