This module guides you through the treatment of hyperpigmentation and teaches you how to successfully utilize the Vivant hyperpigmentation line at home and in office. Due to this, we now recommend using mandelic acid products with a Vivant retinoid like DermaRenu or DermaGel interchangeably with a retinoid-free hydroquinone prescription for the treatment of melasma. Please be aware that hydroquinone must be cycled on and off every three months. When cycling off hydroquinone, patients will continue using their Vivant products for maintenance purposes. Hello, today we are treating a Fitzpatrick 5, 46-year-old female with melasma. We will start her with a skincare consultation followed by a melasma facial.
As you begin your skincare consultation, provide your patient with an at-home starter hyperpigmentation regimen. You can also perform a hyperpigmentation facial the same day. Within a month of your patient starting their at-home hyperpigmentation regimen, we highly recommend scheduling them for a propyl.
Propyl treatments will take your patient's results to the next level since hyperpigmentation can be hard to treat. especially melasma. Repeat once a month. We recommend six to eight propeels for full results. Microdermabrasion and dermaplaning are helpful in-office treatments.
For patients with resistant skin or patients who are Fitzpatrick's 1 through 3, we recommend microdermabrasion and dermaplaning with Vivant 20% glycolic acid peel to minimize scarring. For patients with sensitive skin, pregnant patients and or patients who are Fitzpatrick's 4-6, we recommend using Vivant 30% Mandelic Acid Peel instead. For Fitzpatrick's 1-6 with hyperpigmentation, we recommend the following at-home regimen. Mandelic Acid 3-in-1 Wash, AM and PM.
Daily Repair Pads, PM. 8% Mandelic Acid Serum, AM. Prescription Hydroquinone used interchangeably with Derma Renew, PM. Elantuan, AM. and Day Treatment Lotion SPF 15, AM and PM.
You can also customize a regimen by referring to the Vivant Guide for at-home regimens. Use Mandelic Acid 3-in-1 Wash to cleanse the skin. Massage for about 2-3 minutes to allow the Mandelic Acid to work.
Rinse thoroughly. Gently wipe the face with 3% Mandelic Acid 3-in-1 Toner starting on the forehead, chin, cheeks, and T-zone, avoiding the eye area. Mix equal parts of Prozyme enzyme with Elantuan to the consistency of a paste. Heat enzyme in steamer blast for a few seconds until warm. Apply enzyme up to the hairline around the eye area, not on the lids, and below the jaw area with a fan brush.
Place damp eye pads on the eyes and a towel covering clothing. Apply steam to the face for 15 minutes. Remove enzyme mixture with a warm damp towel or gauze and pat the face dry.
This treatment facilitates the removal of dead skin, allowing for better absorption of products. Apply 8% mandelic acid and massage into the skin. Apply Day Treatment Lotion SPF 15 to protect from the sun's harmful rays.
Hyperpigmentation Product Line. Cleansing milk is our extremely gentle, moisture-rich formula fortified with antioxidant grapeseed oil to promote healthy cell function and replenish the skin. It is our preferred cleanser when treating patients with melasma and very dry skin. Treating pigmented skin with as much mandelic acid as possible is ideal for more consistent results. This is a plus when clients don't have dry skin.
Mandelic acid washes Vivant's antioxidant and antibacterial cleansing gel, formulated to promote even tone and support healthy skin renewal. It's mild enough that it will not overstimulate melanocytes, which can trigger post-inflammatory hyperpigmentation. Hyperpigmentation Toners Toners prepare the skin for deeper absorption of acne-fighting correctors by stripping the skin of oils, dirt, makeup, and bacteria, plus any films left behind by cleansers. Clinical mandelic acid toners are safe for all skin tones and types. These toners are ideal for Fitzpatrick's 4 to 6 due to mandelic acid's non-irritating nature.
For dry skin with melasma, we suggest Skin Nourishing Toner, a 1% mandelic acid, lipid, and antioxidant-rich clarifying formula for even the most sensitive skin. Daily Repair Pads, featured in Vogue magazine, are alcohol-free toner pads. These textured mandelic and salicylic acid pads offer an easy solution for patients who are less inclined to use a spray toner on cotton pads.
These are great for combination skin. For oily skin we offer our 3% Mandelic Acid Toner, a multitasking essential that refined surface texture, promotes even tone, and prime skin for more efficient absorption of corrective serums for accelerated results. Hyper Pigmentation Correctors.
8% Mandelic Acid Serum is a synergistic water-based formulation. Safe for all skin tones and types. It not only evens out skin tone, but it is also an anti-inflammatory and antioxidant serum that enhances the results that patients will obtain with the use of hydroquinone. It does not have to be cycled on and off, so it becomes the perfect maintenance serum for anybody suffering from melasma.
15% Mandelic Acid Serum is our most aggressive morning serum in an alcohol base. It is perfect for oily skin and skin already acclimated to the 8% mandelic acid serum. Retinopropionate is our highly effective patented retinoid.
Retinopropionate exfoliates pigment sitting in the deeper layers of the skin so that it can be treated on the surface. We recommend using our retinoid correctors at night. DermaRenew is a combination of retinopropionate and peptides. The combination of retinoid plus peptides works very well for patients with inflammation. or papulopustular rosacea, since peptides help with inflammation and redness.
Dermagel is a combination of retinopropionate and lactic acid in a water base. It is the retinoid starter for most people with acne unless you have a case of very resistant skin. Along with DermaRenu, Dermagel is safe for all Fitzpatricks.
Hyperpigmentation hydrators Vivant hydrators are non-comedogenic and are safe on all skin types and tones. Most of the time we recommend a daytime moisturizer and no nighttime moisturizer to prevent over-moisturizing acne-prone skin. This would apply to very oily acne-prone skin or for patients that live in a humid climate.
In cases of dry acne or for patients who live in a dry climate, they can moisturize the skin as needed. Always aware that we desire the micro-exfoliation that comes with slight dryness and micro-peeling the first 30 to 60 days of treatment. Total O is our hydrator with whole leaf aloe vera and hyaluronic acid that can be layered under any other Vivant moisturizer for extra hydration.
It aids with redness and inflammation. Total O can be stored in the refrigerator for a cooling effect on the skin before going to bed. Alantoine is our calming lightweight moisturizer since it contains the ingredient alantoine, which in nature is derived from comfrey leaves and has a sedating effect on the skin.
Elantuan works very well when treating dry, sensitive, and chapped skin. Hyperpigmentation Peels 20% glycolic acid is a non-buffered treatment, ideal for resistant and or Fitzpatrick's 1-3. This micro peel is perfect as a lunchtime special since it does not require downtime and only one layer of the product is needed. 20% glycolic acid peel may also be used as a prep before extractions.
As a mini peel, or as a microdermabrasion or dermaplaning treatment booster. This peel must be neutralized with water after a couple of minutes. 30% mandelic acid is a non-buffered treatment, less aggressive but equally effective to glycolic acid.
Ideal for sensitive Fitzpatrick's 1-3 and safe for Fitzpatrick's 4-6, 30% mandelic acid may be used as a prep before extractions, as a mini peel, or as a microdermabrasion or dermaplaning treatment booster. This peel is self-neutralizing and can be left on the skin until the next day. 30% mandelic acid is a pregnancy-safe peel.
Hyperpigmentation SPFs Day Treatment Lotion SPF 15 is our universal moisturizer, formulated with Zinc Ferment and SPF 15. This 2-in-1 lotion provides the needed sun protection for daily home-to-car-to-work-and-back-home cycle of limited sun exposure. Day Treatment Lotion SPF 15 is not intended for prolonged sun exposure, especially for photosensitive patients using retinoids and acids for hyperpigmentation treatment. Mineral SPF 30 is Vivant's pregnancy-safe and coral reef-safe sunscreen recommended for prolonged sun exposure.
Since this product has a high content of zinc, Patients may experience slight tingling when starting on it. We recommend to always apply over a Vivant moisturizer to minimize the tingling sensation that the sunscreen may cause. Hyperpigmentation enzymes.
Prozyme enzymes are an essential part of hyperpigmentation facials or as part of a consultation day in office treatment. Vivant enzymes prep the skin for the recommended at-home treatment and will mildly exfoliate it, leaving it bright and glowy. To perform our enzyme treatments in office, you will need a steamer.
Elantuan is used as an activator for both our enzyme treatments. Prozyme Light is the milder of the two enzymes and its active ingredient is bromelain. Prozyme Light is recommended for sensitive skin.
