Nursing Lecture Notes: Obstetrics Core (GPT-POM)

Introduction

  • Presented by Bonita Gracia, RN, aimed at nursing students and those preparing for the nursing board exam.
  • Topic: Obstetrics core or GPT-POM - important for determining pregnancy outcomes.

GPT-POM Components

  • Gravida: Number of times a woman has been pregnant, including:
    • Previous pregnancies
    • Current pregnancy
    • H-mole (molar pregnancy)
  • Parity: Number of pregnancies reaching age of viability (20 weeks)
  • Term: Number of full-term deliveries (37-42 weeks)
  • Preterm: Deliveries less than 37 weeks but more than 20 weeks
  • Abortion: Pregnancies less than 20 weeks, including miscarriages and H-moles
  • Living: Number of alive births, counting multiples individually
  • Multiple: Number of multiple pregnancies (e.g., twins, triplets)

Detailed Explanation

Gravida (G)

  • Includes H-mole, a non-viable pregnancy where placental tissue develops abnormally.

Parity (P)

  • Count deliveries reaching 20 weeks.

Term (T) & Preterm (P)

  • Term: 37-42 weeks
  • Preterm: Less than 37 weeks but more than 20 weeks

Abortion (A)

  • Pregnancies less than 20 weeks, includes H-mole.

Living (L) & Multiple (M)

  • Living: Count each child in multiple births.
  • Multiple: Count as one.

Examples

Example 1: Mrs. Sharon

  • Currently pregnant, has 2 previous pregnancies at 38 weeks.
  • Obstetric Score: Gravida 3, Parity 2, Term 2, Preterm 0, Abortion 0, Living 2, Multiple 0.

Example 2: Twin Pregnancy

  • Pregnant with twins, 1 previous full-term twin pregnancy.
  • Obstetric Score: Gravida 2, Parity 1, Term 1, Preterm 0, Abortion 0, Living 2, Multiple 1.

Example 3: Single Child

  • Pregnant with twins, has a healthy 4-year-old at 37 weeks.
  • Obstetric Score: Gravida 2, Parity 1, Term 1, Preterm 0, Abortion 0, Living 1, Multiple 0.

Example 4: IUFD Case

  • 3 past pregnancies including an IUFD, currently pregnant.
  • Obstetric Score: Gravida 4, Parity 3, Term 2, Preterm 1, Abortion 0, Living 2, Multiple 0.

Example 5: Complex Case

  • Multiple miscarriages, previous IUFD, currently pregnant.
  • Obstetric Score: Gravida 7, Parity 4, Term 3, Preterm 1, Abortion 2, Living 2, Multiple 0.

Key Points

  • Current pregnancy only included in Gravida until delivered.
  • Multiple pregnancies count as one; living counts each.
  • IUFD (Intrauterine Fetal Death) is not an abortion.
    • IUFD occurs at 20+ weeks; Abortion occurs before 20 weeks.
