Types of Insulin Plans for Diabetes Management

Goal of Insulin Plans

  • Match the body's natural insulin release before diabetes
  • Use a combination of insulin types

Types of Insulin

  • Rapid-Acting Insulin

    • Examples: Humalog, Novolog, Epidra
    • Onset: Starts working within 15 minutes
    • Peak: Works hardest 30 to 90 minutes after injection
    • Duration: Out of the body in 3 to 4 hours
    • Functions:
      • Covers the rise in blood sugar after eating carbohydrates
      • Can be dosed after meals for toddlers to avoid food struggles and low blood sugar
      • Used for correcting high blood sugar at mealtime and bedtime, spaced 3 to 4 hours apart
    • Caution: Avoid hot showers/baths 1-2 hours after injection to prevent fast absorption
  • Long-Acting Insulin

    • Examples: Lantus, Levemir
    • Onset: Starts working in 1 to 2 hours
    • Peak: Levels off and remains consistent for up to 24 hours
    • Duration: Out of the body in 24 hours
    • Functions:
      • Maintains a steady insulin level in the body when not eating carbohydrates
      • Never mix with other insulins
      • Usually given once a day at the same time
      • Requires additional rapid-acting insulin during meals
    • Typical regimen: 4 to 6 injections daily
  • Intermediate-Acting Insulin

    • Examples: Humulin N, Novolin N (NPH insulin)
    • Appearance: Cloudy due to added protein for slow absorption
    • Onset: Starts working 2 to 4 hours after injection
    • Peak: Works hardest 4 to 12 hours after injection
    • Duration: Out of the body in 12 to 18 hours
    • Functions:
      • Covers mid-morning snack, lunch, mid-afternoon when taken at breakfast
      • Requires additional injection at supper for bedtime snack and overnight insulin
      • Can be mixed with rapid-acting insulin for meals
    • Typical regimen: 3 to 4 injections daily, with scheduled meals and snacks
