Canadian Medical Pathways for IMGs

Jun 16, 2025

Overview

The lecture outlines four main pathways for international medical graduates (IMGs) to become certified doctors and practice medicine in Canada, covering key steps and requirements for each route.

Pathway 1: Traditional Permanent Residency Route

  • Complete a medical degree from a World Health Organization-recognized college.
  • Verify medical degree and credentials through physiciansapply.ca and Medical Council of Canada (MCC).
  • Pass MCC Qualifying Examination Part 1 (MCCQE1) and National Assessment Collaboration (NAC) OSCE exam.
  • Apply for Canadian permanent residency (PR) via a points-based system from your home country.
  • Once PR and exams are complete, apply for residency positions via the CARMS website.
  • After completing residency and Royal College exams, obtain a provincial medical license to practice as a consultant.

Pathway 2: Job Offer & Canadian Experience Class

  • Apply for a paid job in Canada (healthcare, research, etc.) to gain Canadian experience.
  • Jobs should qualify you for Canadian Experience Class, helping PR application.
  • Once in Canada, verify credentials, pass MCCQE1/NAC exams, and apply through CARMS like Pathway 1.
  • Benefit from Canadian experience, networking, and potential recommendation letters to improve residency match chances.

Pathway 3: Atlantic Immigration Program

  • Focus on jobs in one of four Atlantic provinces: New Brunswick, Newfoundland and Labrador, Prince Edward Island, or Nova Scotia.
  • Obtain PR and build connections in the province, increasing competitiveness for local residency programs via CARMS.
  • After residency and certification, proceed to licensing and practice.

Pathway 4: Clinical Fellowship Route

  • Complete postgraduate residency training in your home country.
  • Apply for a clinical fellowship at one of Canada's top 17 universities.
  • Strengthen application with voluntary work, research, and publications during residency.
  • After fellowship, either apply for Canadian residency matching or explore alternate pathways to practice directly as a consultant.
  • Some alternate pathways may exempt you from repeating residency or taking MCCQE1.
