Setting Up the V60 NIV Ventilator

Introduction

  • Video is for trained doctors and nurses familiar with respiratory failure
  • Aims to provide a reminder on setting up the V60 NIV ventilator used in Addenbrooke's emergency department

Hardware Overview

  • Power and Battery
    • On button, battery indicator light
    • Battery lasts several hours; keep plugged in when in use
  • Connections
    • Power cabling, oxygen tubing connects to main ports in the wall
    • Oxygen cylinder at base for transfer
    • Patient circuit and pressure monitor tubing attachments
  • Touchscreen and Dial Pad
    • Touchscreen device with dial pad backup
  • Specialized Circuit
    • Comes with an HME filter
    • Pressure sensor tubing and T-piece for nebuliser attachment

Starting the V60

  • Device has multiple modes and options
  • Initial Setup for BiPAP (Type 2 Respiratory Failure)
    • Turn on device; it resumes last settings
    • Press standby to pause
    • Select BiPAP mode: choose ST (spontaneous timed)
  • Setting Screen
    • IPAP: Start at 15 cm H2O
    • EPAP: Start at 5 cm H2O
    • Rate: Backup rate for periods of apnea
    • Inspiratory Time (I-time): 1 second
    • Rise: Start at middle setting (e.g., 2) for comfort
    • RAMP: Not typically used in emergency settings
    • Oxygen: Set FIO2 and activate

Operation

  • Connect circuit to patient for breath detection and triggering
  • Alternatively, press restart

Post-Setup

  • Gradually increase inspiratory pressures over 30 minutes to 20-30 cm H2O
  • Continuous monitoring required
