in this video we're going to look at the difference between supra bony pocket and infra bony pocket and it's really simple so supra bony let's look at this word supra means above and bony comes from the word bone so something is above the bone what what is above the bone in this case if we look this is the junctional epithelium and what we know about the je or the junction of epithelium is that when we probe the base of the sulcus or the bottom of the probe will always hit the junctional epithelium and so if you look at this picture here the junctional epithelium is above or supra to the bone so that's a super bony pocket when someone or when the junction of epithelium is above the bone it's considered super bony pocket so this pocket over here is known as a super bony pocket so this could be like a six millimeter pocket or a seven millimeter pocket and that pocket is known as a supra bony pocket because the junctional epithelium is supra or above the bone when does this happen this happens when we have horizontal bone loss so anytime there's horizontal bone mass this would happen let's look at infrabony so infra means below and throughout the bone so below the bone so again when we look at the junction of epithelium look at it in relation to the bone in relation to the crust of that bone and we can see in this case we have an infrabony pocket because the junctional epithelium is infra or inferior to infra comes from inferior below the bone so when the junction of epithelium is below the bone the crust of the bone it's known as an infrabony pocket so here we can have again a seven or eight millimeter pocket and this pocket is known as an infra bony pocket why because the junctional epithelium is apical or below the bone
