Supine Spinal Immobilization Practical Assessment Notes

Introduction

  • Purpose: Evaluate ability to secure a patient to a long backboard.
  • Patient's vitals are stable.
  • Instructor's role: Provide instruction and ensure proper procedure.
  • Assistant's role: Help with the immobilization process.
  • Equipment: Review before starting.

Initial Steps

  • BSI (Body Substance Isolation): Confirm scene safety.

Procedure

Head and Neck

  • Direct partner to place head in an inline neutral position.
  • Maintain C-spine stabilization.

Circulation, Motor, and Sensation (CMS) Check

  • Upper Extremities:
    • Check bilateral pulses.
    • Ask patient to squeeze fingers and identify touched fingers.
  • Lower Extremities:
    • Remove shoes.
    • Assess pedal pulses.
    • Ask patient to push and pull against hands.
    • Identify touched toes.
  • Confirm pulses, sensation, and motor function are intact.

C-Collar Application

  • Size and select appropriate C-collar.
  • Preform collar and apply without excessive neck movement.
  • Ensure collar fit by asking patient to open their mouth slightly.

Log Roll and Positioning

  • Log roll patient with assistance on instructor's count.
  • Check the patient's back for deformities or wounds.
  • Position the long board.
  • Roll patient flat onto the board.

Securing the Patient

  • Move patient up and over on the board in one motion.
  • Secure torso with straps:
    • Under arms, tighten straps, and reposition arms.
  • Check need for padding and apply if necessary.
  • Secure head, legs, and feet.

Reassessment

  • Reassess CMS:
    • Upper Extremities: Check pulses, finger identification, strength.
    • Lower Extremities: Check pedal pulses, toe identification, strength.
  • Confirm intact pulses, sensation, and motor function.

Conclusion

  • Ensure all steps are completed and patient is fully secured.
  • Additional actions not required.
