Introduction to Rotary Endodontic Files
- Rotary files are engine-driven instruments used for root canal shaping and are more efficient than manual files.
- The ProTaper Gold system is commonly used, but clinicians must understand each file system's unique instructions.
Evolution of the ProTaper System
- The original ProTaper launched in 2001, followed by ProTaper Universal in 2006 (conventional nickel-titanium).
- ProTaper Next (2013) used M-Wire and has different operating settings.
- ProTaper Gold (2014) uses heat-treated gold wire for improved flexibility and resistance to cyclic fatigue.
ProTaper Gold Instrument Family
- The system has seven files: three shapers (SX, S1, S2) and five finishers (F1–F5).
- SX is an orifice opener; S1 (purple) and S2 (white) shape coronal and middle thirds.
Shaper Files: Uses and Techniques
- SX opens the orifices and shapes the coronal third with a brushing motion.
- S1 and S2 cut in the coronal and middle thirds only, used with a "brush and follow" motion, requiring a prior glide path.
Finishing Files: Uses and Techniques
- Finishers (F1–F5) shape the apical third and follow ISO tip sizes: F1 (size 20), F2 (25), F3 (30), F4 (40), F5 (50).
- These are used with a "kiss the apex" technique and should not remain long in the canal to prevent procedural errors.
Sequence of Use and Clinical Protocol
- Requirements: proper access, glide path (size 20K file), and correct equipment (X-Smart Plus at 250 rpm, 4 Ncm torque).
- Sequence: orifice opening (SX or Gates Glidden), verify working length, establish glide path, S1 and S2 for shaping, F1–F5 as needed for apical preparation.
- Inspect debris on apical flutes to determine when shaping is complete.
Irrigation and Maintenance
- Continuous irrigation with sodium hypochlorite is essential between each file.
- Patency should be checked and files cleaned to maintain efficiency.
