Peripheral IV Insertion Overview
Preparation
Confirm and follow hospital protocols.

Perform hand hygiene.
Finding a Vein and Hair Trimming
Identify the vein for IV insertion (e.g., hand vein).
Trim hair if necessary for better tape adherence and patient comfort.
Avoid using a straight razor.
IV Insertion Procedure
Apply Tourniquet:
Choose the vein and apply a tourniquet.
Disinfect the Site:
Use Chloraprep or similar disinfectant, start at the center and work outward for about 30 seconds.
Allow the site to completely dry.
Insert the IV:
Stabilize the vein by pulling on the skin below the cleansed area.
Hold the IV at a 20-degree angle, bevel up.
Insert following the vein's natural angle.
Check for blood return in the chamber.
Advance the cannula slightly more.
Post-Insertion
Remove Tourniquet:
Apply pressure at the site to prevent bleeding.
Needle Safety:
Activate the safety button to retract the needle.
Connect Access Device:
Securely twist the access device onto the cannula.
Check for blood return and flush the IV with a push-pause technique using 3-5 cc of fluid.
Ensure there is no infiltration (no swelling at the site).
Cap Usage
Depending on the cap type (neutral, positive, or negative pressure), adjust the procedure:
Neutral cap: Clamp off the line.
Positive pressure: Flush, disconnect, then clamp.
Negative pressure: Flush, clamp, then disconnect.
Securing the IV
Apply dressing and protective cap:
Ensure firm pressure and proper placement on the skin.
Use notched dressing over the cap.
Remove excess edges and ensure tape is secure.
Form the tubing into a J shape to prevent tension.
Use additional tape to secure the tail of the IV line.
Record insertion details (date, other required documentation).
Final Steps
Dispose of waste as per facility protocols.
Remove gloves and perform hand hygiene.
Document the procedure.
