Overview

This lecture covers the principles, history, and practical procedures for using rotary endodontic files, focusing on the ProTaper Gold system for cleaning and shaping root canals in molars.

Rotary Files in Endodontics

  • Rotary files are engine-driven, mechanical instruments (rotary or reciprocating) used for shaping root canal systems.
  • The ProTaper Gold system is widely taught and used; always follow manufacturer-specific instructions regarding speed, torque, and usage.
  • Nickel titanium (NiTi) is preferred for rotary files due to strength and flexibility, allowing navigation of curved canals.

History and Metallurgy of ProTaper Systems

  • ProTaper system launched in 2001 (conventional NiTi), rebranded as ProTaper Universal in 2006 with added features.
  • ProTaper Gold (2014) uses heat-treated M-Wire for greater cyclic fatigue resistance and flexibility versus earlier versions.
  • ProTaper Next is a different system with distinct settingsβ€”do not confuse with ProTaper Gold.

ProTaper Gold Instrument Family and Usage

Shapers (SX, S1, S2)

  • SX: Short, fat orifice opener (19mm), used for coronal shaping.
  • S1 (purple): Prepares coronal third; tip size ISO 18.
  • S2 (white): Prepares middle third; tip size ISO 20.
  • Shapers use a "brush and follow" motion, shaping upper canal thirds, not the apical third.

Finishers (F1–F5)

  • F1 (yellow): Tip ISO 20, shapes apical third.
  • F2 (red): Tip ISO 25.
  • F3 (blue): Tip ISO 30.
  • F4 (black): Tip ISO 40.
  • F5 (yellow): Tip ISO 50.
  • Finishers use a "kiss the apex" technique; do not keep at working length for long.

Clinical Sequence for Cleaning and Shaping

  • Establish straight-line access and smooth glide path (confirmed with size 20 K-file).
  • Use SX/gates glidden to open orifices, then S1 and S2 for shaping coronal and middle thirds.
  • Irrigate and check canal patency after each file.
  • Use F1–F5 as needed based on debris in apical flutes; proceed to larger files if shaping is incomplete.
  • Continuously clean debris from file flutes and irrigate with sodium hypochlorite for disinfection.

ProTaper Gold Use: Practical Points

  • Operate at 250 rpm and 4–5 Ncm torque using an endodontic motor (not a slow handpiece).
  • Use light pressure; let the file do the work.
  • Irrigate frequently, aiming for 30 minutes total sodium hypochlorite contact time during procedure.
