Perineal Care Procedure for Ms. Jones

• Two chucks, four washcloths, privacy blanket, towel, basin, soap, and gloves.

Preparation Steps

1. Table Setup: Place barrier on over-bed table and arrange supplies.
2. Fill Basin: Use a paper towel to turn on water, ensuring it is warm. Confirm temperature with Ms. Jones.
3. Privacy: Place a privacy blanket over Ms. Jones to protect her privacy and keep her warm.
4. Chucks Preparation: Unfold and roll two chucks, clean rolls toward CNA.

Procedure for Changing Chucks

• Left Side:
  • Ask Ms. Jones to scoot and roll onto her left side.
  • Roll the dirty chucks towards the patient, tuck under her hip, unroll clean chucks.
• Right Side:
  • Ask Ms. Jones to scoot and roll onto her right side.
  • Remove soiled chucks and unroll clean one.

Cleaning Process

Front Cleaning

• Uncover Area: Roll blanket to expose the perineal area, cover legs with a towel.
• Use First Washcloth:
  • Clean inside middle (top to bottom), outside labia, skin folds.

Back Cleaning

  • Clean anal area and buttocks.
• Rinsing and Drying: Repeat washing steps for rinsing and drying.

Final Steps

• Change Pad: Replace soiled chucks with clean ones as done previously.
• Cleaning Basin:
  • Dump water, rinse, dry inside and outside using paper towels.
  • Return basin to storage with paper towel assistance.
• Remove Gloves and dispose of all leftover materials.
• Comfort and Closure:
