Mechanism of Action

  • Alpha 1 Receptors:
    • Part of the sympathetic (fight-or-flight) nervous system
    • Target smooth muscle in the body
    • Effects include:
      • Vasoconstriction
      • Contraction of detrusor muscles in the bladder
      • Contraction of the prostate
  • Antagonist Action:
    • Tamsulosin blocks/inhibits alpha 1 receptors
    • Effects of Tamsulosin:
      • Vasodilation
      • Relaxation of detrusor muscles
      • Relaxation of the prostate
    • Improves urinary flow

Clinical Uses

  • Primary Use:
    • Treatment of symptoms of Benign Prostatic Hyperplasia (BPH)
      • Symptoms include nocturia, hesitancy, pain during urination, dribbling
  • Off-Label Use:
    • Urinal stones

Limitations

  • Not commonly used for hypertension treatment due to better options available.

Side Effects

  • Common Side Effects:
    • Hypotension (dizziness, drowsiness, headache)
    • Orthostatic hypotension (common in elderly)
      • Sudden BP drop during position changes
  • Monitoring and Management:
    • Monitor intake/output
    • Advise elderly to sit on the edge of the bed before standing to reduce syncope risk
