Understanding Gingivitis Categories and Treatment

Apr 27, 2025

Gingivitis Categories Overview

Introduction

  • Gingivitis is inflammation of the gums.
  • Plaque is often a cause, leading to what is known as plaque-induced gingivitis.

Categories of Gingivitis

  1. Plaque-Induced Gingivitis on an Intact Periodontium

    • Clients with inflamed gums and possible bleeding and redness.
    • Radiographs show intact bone level; no bone loss has occurred.
    • Classified as a gingivitis client with an intact periodontium.
  2. Gingivitis on a Reduced Periodontium in a Non-Periodontal Client

    • Presence of gum recession but intact bone level.
    • Recession possibly due to previous orthodontic treatment (e.g., braces).
    • Classified as having gingivitis without periodontitis.
    • Recession is present, but no bone resorption is noted, making this a reduced periodontium.
  3. Gingivitis on a Reduced Periodontium in a Successfully Treated Periodontitis Patient

    • Client has a history of periodontitis, but is now stable.
    • Bone loss is evident, but stable without further resorption.
    • Presence of gingivitis symptoms including bleeding and inflammation.
    • Classified as having gingivitis on a reduced periodontium in a previously treated periodontal patient.

Key Observations

  • Recession vs. Bone Loss:
    • Recession does not always indicate periodontitis.
    • Bone loss previously treated can indicate a stable periodontitis patient now showing gingivitis.

Conclusion

  • Correct classification is crucial for identifying the appropriate treatment and understanding the patient's periodontal health.
  • Understanding the history of orthodontic treatments and previous periodontal conditions is important for accurate diagnosis.
