TPN Infusion Preparation and Setup

Introduction

  • Purpose: Instructions on how to prepare and start a TPN (Total Parenteral Nutrition) infusion.

Supplies Needed

  • TPN bag
  • Empty syringe
  • Multivitamins
  • Tubing
  • Pump and pump key
  • Hand sanitizer
  • Alcohol wipes
  • Sharps container

Preparation Steps

  1. Sanitize Hands: Ensure hands are properly sanitized.
  2. **Prepare TPN Bag with Multivitamins: **
    • Wipe the top of each vial with alcohol.
    • Attach needle to empty syringe.
    • Remove needle cap and add air into the syringe.
    • Push air into the vial, turn it upside down, and draw out 5 ml of liquid.
    • Repeat the process to draw a total of 10 ml of multivitamins.
    • Recap the needle and wipe the injection port of the TPN bag.
    • Inject the multivitamins into the TPN bag, ensuring needle goes in straight.
    • Mix by gently rocking the bag; the solution should be light yellow without any solid specks.

Setting Up Tubing and Pump

  1. **Tubing: **
    • Remove blue cap from the tubing.
    • Spike the TPN bag using a forceful twisting motion.
    • Raise the TPN bag to allow the solution to fill the tubing.
    • Ensure air filter is placed vertically.
    • Fill tubing all the way to the end of the cap.
  2. **Pump: **
    • Insert the battery and turn on the pump.
    • Load the tubing into the pump, ensuring it clicks into place.
    • Use the pump key to secure the tubing.
    • Reset reservoir volume if necessary by pressing 'Next' and 'Enter'.

Connecting to Patient

  1. **Flush Line: ** Ensure the PICC line or central line is flushed before connecting.
  2. **Attach Tubing: ** Connect the end of the tubing to the PICC line or central line.
  3. **Start Infusion: **
    • Hang the TPN bag and start the pump.
    • Press the 'Stop/Start' button and confirm by pressing 'Yes'.
    • Ensure the main screen indicates 'Running'.
