Transcript for: Steps for Modified Bed Bath Procedure
[Narrator] Skill 11,
gives modified bed bath, face and one arm, hand, and underarm. For the purposes of this demonstration, we've raised the head of the bed, but you wouldn't do that
for the actual skill exam. Step one, address client by name and introduces self to client by name. Step two, explains
procedure speaking clearly, slowly, and directly, maintaining face-to-face contact whenever possible. Step three, privacy is provided with a curtain, screen, or door. - Knock, knock, hi, Misses
Jones, my name is Mark. I'm gonna be your student nurse aid today, and today we're gonna give you a bed bath. Make sure the bed is in a
low and locked position. Call light's within reach. Privacy curtain is pulled. - [Narrator] Step four, before washing, checks water temperature
for safety and comfort and asks the client to
verify comfort of water. Step five, puts on clean
gloves before washing client. Step six, removes gown and places directly in soiled linen container
while ensuring client's chest and lower body are covered. - Alright, Misses Jones,
I'm gonna grab some water. Turn the cold on first and then the warm. Turn the hot off and then the cold. Okay, Misses Jones, can you
check this water for me? - Feels fine. - Perfect. Alright, Misses Jones, if you can, put your arms underneath
the covers for me. Thank you. I'm gonna pull this arm out first. Okay. And now pull this arm out. Now I'm going to grab
by the patient's chin, going from the side I'm going
to grab hold of the gown and then I'm going to pull out. As I'm pulling out, I want
to walk that gown around my hand making sure that I keep it away from my body not to expose myself. And dispose. - [Narrator] Step seven,
beginning with eyes, washes eyes with wash cloth, no soap, using a different area of the
washcloth for each stroke, washing the inner aspects to outer aspect then proceeds to wash face. Step eight, dries face with
dry cloth, towel, or washcloth. - Alright. First thing, we're gonna put
the towel underneath your head. Can you lift your head up for me, please? And we wet the washcloth, and then I'm gonna take this washcloth and we're gonna fold it into fours. There's gonna be some tabs here and you're gonna hold the
tabs here at your thumb, and every time that you
need to switch to a new part of the washcloth, you're
going to lift up your thumb, fold it over your hand,
and reposition your hand. You'll do this every time that you switch positions of the cloth. So making sure that
it's folded into fours, tab is gonna be at my thumb, and gonna start by washing the eyes. We're gonna start from the
inter canthus, so by the nose, and we're gonna do one swipe and go out. Lift up your thumb, flip that washcloth, and reposition the hand,
then the other eye, inner canthus, out. Lift up your thumb, flip the washcloth, reposition your hand. The next thing we're gonna
do is it's a three motion. So we're gonna start from the forehead, go across the forehead down by the ear, up the cheek, up the nose, down the nose, back across the cheek, across the jawline and over the mouth. Lift up your thumb,
reposition the washcloth, and then run the other side
starting at the forehead, going down down to the ear,
up to the bridge of the nose, down the bridge of the
nose, across the jawline and across the mouth, and then dispose. Then we're going to dry,
and you're gonna dry in a blotting fashion making
sure that you use a new portion of the towel for each part of
the face, especially the eyes. Okay. - [Narrator] Step nine, exposes one arm and places cloth towel underneath arm. Step 10, applies soap with wet washcloth. Step 11, washes fingers
including fingernails, hand, arm, and underarm,
keeping rest of body covered. Step 12, rinses and dries
fingers, hand, arm, and underarm. Step 13, moves body gently and naturally avoiding force and overextension
of limbs and joints. - Alright, Misses Jones, can
you lift your head for me? I'm gonna expose just one arm. Can you lift your arm out for me? Take the towel, put it all
the way up to the armpit. Starting with your washcloth, getting it wet, one pump of soap, and with this part, you don't have to fold the washcloth into fours. We're gonna start with the fingernails making sure that you swipe that washcloth under each
individual fingernail. Then wash the finger, the palm, and the back of the hand. Now we're gonna go up to the forearm area. Once we go to the forearm, we
can never go back to the hand. Making sure that you're washing
all surfaces of the arm, and now we're gonna go up to the upper arm making sure that once
we go to the upper arm, you're never gonna go back
down to the lower arm, and an easy way to remember this is once you passed a joint,
you never go back. Then we get the shoulder,
and then what I like to do is raise that patient's arm
up and then do one swipe making sure that you get all the way in to their axillary, or their armpit, and disposing. Lower your arm. We're gonna do a rinse washcloth. You're gonna do the same thing as you did with the soapy washcloth, underneath each individual fingernail, each finger, the palm of the hand, and the back of the hand. Then we're gonna move up to the forearm, making sure that I don't
go back to the hand, then the upper arm, making
sure that I don't go back to the forearm, and getting
all parts of the upper arm, getting the shoulder, one
swipe down into the armpit, and dispose. Now we're going to dry
each individual finger including underneath the fingernail, the palm and the back of the hand, and then the patient can lay
their arm down and just wrap the towel around and blot
all the way up the arm, making sure that you get the shoulder, lifting up their arm, and making sure that you get their armpit. Put your arm down for me, and dispose. - [Narrator] Step 14,
puts clean gown on client. Step 15, empties or
rinses and dries basin. Step 16, places basin in
designated dirty supply area. Step 17, disposes of linen
into soiled linen container. Step 18, avoids contact between student clothing and used linens. - Alright, Misses Jones, can you pull this arm out for me as well? We're gonna put your gown on. I'm gonna place my hand through the gown, grab the patient's hand, pull the gown up. Relax that down. Same thing, hand through the gown, grab the patient's hand and pull it up. At this time, you're
gonna grab hold of where the neck of the gown is and
you're gonna slowly pull down the sheet to make sure that
you don't expose the patient, and cover them up, dispose of my equipment. I'm going to empty, rinse, empty, and dry. - [Narrator] Step 19, removes
and disposes of gloves without contaminating self into waste container and washes hands. Signaling device is within reach
and bed is in low position. - Wash my hands. Let's grab your call light. Call light's within reach. Bed is in a low and locked position. Privacy curtain back. Wash my hands, and skill complete.
