So here is your Alaris channel with your primary fluids channel. I already have some primary fluids, so I just have LR that is primed through the primary tubing ready to go. I have my Alaris key for the PCA. I've got my PCA channel which I'm going to put my PCA syringe into. And then I also have My Monoject syringe that comes from pharmacy.
I've got it, it's blue because it's going to allow us to see easier how you're supposed to prime or to where you will prime the analgesia through. And then I also have my Monoject syringe which is going to go inside of this barrel, just twist like that. I also have my PCA tubing. What I'm going to do is I'll take my MonoJect syringe and I'm going to attach it to the long end of the PCA tubing.
Just make sure that you've got all your PCA tubings or the clamps engaged because now once we've got it attached we're going to hook it up into our PCA module to prime. Okay so now that we have our syringe connected to our tubing we'll go ahead and we've turned on our pump And what we need to do is open up our little plastic case that encloses the PCA syringe. And so you'll use your key that you get from the Pixus and you need to turn it counterclockwise until it's to unlock and then it'll pop this open.
Once that's opened up, there are a few steps to insert the syringe into here. You need to take this little glass piece on the bottom and kind of pull it out towards you and then over. You will then Put your syringe, line it up here underneath this little lip so it should slide in kind of nicely right there like that. Okay and then you'll take this plastic piece come back towards you and then down so it holds it in place. You'll then twist this part up here over to the left so that it opens up these clamps up here and then you'll lower it down onto the top of the barrel of your syringe and then close it up like that.
Now we're going to close the case and turn our key to program. We'll go ahead and hit channel select. It'll ask us to confirm the time always. We just want to make sure that time is correct. We'll select monoject 35, so that's what we have in here.
Confirm. We'll select morph green. And this is asking for our specific order, so I'm just going to say PCA under 65. And it's just confirming our dose, so 5 milligrams in 1 mil.
It's always going to give us a clinical advisory that we need a second RN to sign off on it. And it's going to, again, give us our drug amount and the new one. And then before you select your infusion mode, we need to prime the tubing. So to do that you have to go to this option button, options, page down, and then prime set with syringe.
So select that and you'll get to this screen with the drops. Go ahead and unclamp all of your tubing. And to prime we're just going to click this button and hold.
Click and hold and you'll start to see that it will begin. counting the amount of mils that is going through the tubing and it's going to automatically stop at two mils. Once at two milliliters, the analgesia should be, if you can see here, just past the Y port or just to the Y port.
If it's not, you can always hit the prime button again to make it go longer, but the long piece of the tubing is should be very close to two mils and so if you just click and hold that prime button and you go to all the way to two mils it should go just to the y port or to the y and as the last piece we need to make sure that we prime the rest of our tubing with our primary fluid so we have our primary fluids up here we're going to attach our primary fluids and just prime the rest of the PCA tubing by gravity. That's the easiest way to do that. So we'll attach our primary fluids to the other side to that other port here.
And then we'll go ahead and release our clamps and we'll go ahead and prime that through so that we've got primary fluids going all the way to the end that is going to attach to our patient. Now that we have our primary fluids in their channel and we've got our PCA analgesia in their channel, let's go ahead and... Programmer infusions will go to, for this patient, they're just getting the PCA dose only. We're going to do PCA dose of 1 mg. The lockout interval is 10 minutes.
Max limit is 15. Concentration is 5 mg per ml, so we're going to confirm all that. We're going to close. The door's already closed, so we're just going to lock the door.
And then we're going to go ahead and click start. For our primary fluids, we'll just hit channel select. Let's just do a basic infusion of 125 of 950 to be infused and that'll go ahead and start infusing.
All right so we've got our fluids, we've got our PCA ready to go, and now if our patient hits their green button you hear that sound they're going to get a little teeny bit of morphine which will be carried to them by the primary fluid once it's passed that way. Be sure to take your key and return to the
